Public Health Department

The Beaumont Public Health Department is continuing its transition into the new millennium. The department has begun to assess the needs of the community as it relates to public health. Community-based activities are becoming the major focus of the department. Recognizing the new role that must be established to ensure a healthy community, the department created partnerships with various public and private entities. The department serves as a training site for students at Lamar University acquiring Bachelor degrees in nursing, nutrition and health promotion, also providing preceptorships for graduate nursing, biology and nutrition students.

The City of Beaumont's Health Department wants citizens to be aware of the signs and symptoms of stroke, which may include:
* Sudden weakness or numbness of the face, arm and leg on one side of the body
* Loss of speech, or difficulty talking
* Dimness or loss of vision
* Unexplained dizziness, especially when associated with any of the above
* Unsteadiness or sudden falls
* Headache (usually severe and of sudden onset)
* Confusion

ADMINISTRATIVE SERVICES

Administrative Services provides guidance and direction to the various divisions of the department through the monitoring of established goals and objectives. The section implements corrective action when deemed necessary. Additionally, data collection and analysis are integral parts of its role.

PUBLIC HEALTH PROGRAMS

Public health programs with the Health Department provide a variety of services, including public health preparedness, which encompasses the investigation and surveillance of infectious diseases including tuberculosis, sexually-transmitted and vaccine-preventable diseases and other reportable diseases. The Public Health Preparedness Division also develops, implements, and exercises plans to respond to man-made and natural disasters.

The Immunization Division offers immunizations to infants, children, adolescents, and adults according to the recommended schedules which have been approved by the Advisory Committee on Immunization Practices.

Sexually-transmitted disease (STD) services are provided in a confidential manner by competent staff who are prepared to assess, diagnose and treat a variety of STDs. Follow-up investigation and surveillance of STDs is carried out by a staff of disease intervention specialists (DIS). All persons accessing services in the STD Clinic will be tested for syphilis, gonorrhea, chlamydia, and HIV.

The Beaumont Public Health Department works in conjunction with the Texas Department of Health in issuing, at a cost, rabies vaccine for post-exposure to potentially rabid animals.

Immunization Clinic

Hours: Monday, 8:00 a.m. - 5:00 p.m.
First Monday of the month: 8:00 a.m. - 6:00 p.m. &
Tuesday - Friday, 8:00 a.m. - 5:00 p.m.
No appointment necessary. Service on walk-in basis.
Fee: $10.00 per child / $15 per adult. Medicaid accepted
Record Replacement: $5 per record
( No one will be denied for inability to pay)
Residency requirement: None

Public Health Preparedness Division

Hours: Monday - Friday: 8:00 a.m. - 5:00 p.m.
To report a reportable condition (including after hours and on the weekends), call 409-839-4208

Sexually Transmitted Disease (STD) Clinic

Hours: Monday - Friday 8:00 a.m. - 4:00 p.m.
No appointment necessary. Service on walk-in basis.
Fee: $20.00 (However, no one will be denied service for inability to pay)
Residency requirement: None
Tuberculosis Clinic

Hours: Monday - Friday 8:00 a.m. - 5:00 p.m.
Appointment necessary
Fee: No Charge
Residency requirements: North Jefferson County

VITAL STATISTICS

Vital Statistics is the registration, preparation, transcription, collection, compilation, and preservation of data pertaining to births, acknowledgements of paternity, deaths, fetal deaths (stillbirths), and data incidental thereto.  We issue certified death certificates on individuals that died in the city of Beaumont and Precincts I and VI.

We can be reached at (409) 832-4000 or (409) 654-3631.  Hours of operation are Monday – Friday, 8:00am – 5:00pm.  Birth and death certificates are issued the same day when you are in by 4:00pm.  Applications are taken until 5:00pm and mailed the next business day.

Confidentiality

Birth Records
Birth records are confidential records for the first 75 years after filing and death records are confidential for the first 25 years. These records may only be issued to a properly qualified applicant upon submission of a written application that fully identifies the records.

Adoption Records
Adoption records are confidential forever and may only be opened upon order of the court which granted the adoption. These records are opened at the State Bureau of Vital Statistics.

Acknowledgment of Paternity Records
Paternity records are confidential.  A certified copy may only be obtained through the State Bureau of Vital Statistics.  Call the Vital Statistics office for more information.

Qualified Applicants
A qualified applicant is the registrant, or an immediate family member (parent, grandparent, or sibling) either by blood or marriage, his or her guardian, or his or her legal agent or representative.  Local, state, and federal law enforcement or governmental agencies and other persons may be designated as properly qualified applicants by demonstrating a direct and tangible interest in the record when the information in the record is necessary to implement a statutory provision or to protect a personal legal property right.  This is demonstrated by written proof.

Payment For Services
Payments that are acceptable for services include cash, personal checks, cashier’s checks, or money orders with proper identification. Payment is to be made to the Beaumont Public Health Department. Personal checks must contain pre-printed name and address. Two-party checks, checks greater than the amount of purchase, and out-of-state checks will not be accepted. Sorry, no credit cards.

Filing Fees
All amendments to birth certificates are processed through the State office in Austin and payable to the State Bureau of Vital Statistics.  Forms available at

HEALTH EDUCATION SERVICES

The Health Promotion and Education Division provides a valuable service to the community through outreach programs geared towards the promotion of good health through health education with a broad range of health topics from wellness to specific health problems, such as diabetes and hypertension.

Internship Program
The Beaumont Health Department’s Health Promotion Division offers internships in conjunction with the Lamar University Health Kinesiology/Family & Consumer Science AP4 Program and Nursing.
